What We Offer
At this time we only service New Castle County Schools
Provider-to-Provider Referral Support
We partner with medical professionals, pediatricians, family medicine clinicians, psychiatrists, and specialty care teams to identify appropriate mental health services for patients. Our team helps facilitate referrals and provides clarity around service options and care pathways.
Patient and Family Referral Assistance
If you or a family member has been referred for therapeutic support, behavioral health services, or counseling, we assist with connecting you to the right provider and explaining next steps in the process, including what to expect.
Care Coordination and Consultation
We collaborate with your existing healthcare team to help align treatment goals, share relevant information with consent, and support continuity of care between medical and behavioral health services.
Personalized Recommendations
Every individual has unique needs. We help clarify service options such as outpatient therapy, psychiatric support, or community wellness resources and offer personalized guidance based on symptoms, goals, age, and preferences.
Medication Management
New at Tranquility Wellness. Through our new partnership with Mindful Haven, we are now able to offer our clients Medication Management!
Comprehensive assessments and medication management services include detailed clinical evaluations to identify and diagnose conditions such as ADHD, Bipolar Disorder, PTSD, anxiety, depression, and other mental health concerns.
What We Provide?
Assessments involve reviewing medical and mental health history, current symptoms, functioning, and treatment goals to ensure an accurate diagnosis and individualized care plan. Medication management is then provided through careful selection, monitoring, and adjustment of medications, with ongoing follow-up to evaluate effectiveness, manage side effects, support adherence, and coordinate care with other healthcare providers to promote long-term stability and well-being.

Why Coordinated Care Matters
Physical and mental health are interconnected. When emotional or behavioral health concerns are present alongside medical symptoms, uncoordinated care can lead to fragmented outcomes, longer recovery timelines, and increased stress for individuals and families.
By bridging medical referral needs with thoughtful mental health support, we help ensure clear communication between providers, smooth transitions between services, appropriate matching to therapy or psychiatric care, and empowered patients and families who understand their options.
Our goal is to support access to care that promotes balance, resilience, and whole-person wellness.
